Scheduling Integrative Cancer Support Appointments

Please follow the steps below to get started. If you have ANY questions before getting started, please call our office and talk with our staff. We want you to feel confident that this is the right next step in your journey.

Steps to get you in fast:

Go to: https://mendintegrative.md-hq.com/

Click “New Patient Registration” at the bottom
Fill out registration form
Click “Sign Me Up”
Continue to step 2

Click “Questionnaires” in your portal
Complete all functional medicine questionnaires thoroughly
Sign the Informed Consent forms (functional health, nutrition, and mental health)
Sign Telemedicine Consent if applicable
Complete all supplements and medication information
Upload any recent labs (last 6-12 months) in the Documents section
Hit “Submit” at the bottom of each questionnaire

Once your portal is completely set up, give us a call and we will assist you in scheduling your appointments. Depending on your acuity, we try to schedule patients with cancer within 1-3 weeks as our schedule allows.

Log in to your portal
Go to “Invoices & Payments” tab
Add your payment method (credit/debit card)
Then click “Make a Payment” to pay the deposit for your appointment.

The $150 deposit reserves your appointment. The remaining balance of $350 will be due at your first scheduled visit.

Cancellation Policy

Your appointments represent significant time we’ve set aside exclusively for your care. We have many patients waiting to be seen.
Late cancellations and no-shows prevent us from serving others who need care and will result in fees.

Cancellations require 3 business days’ notice. If we don’t answer, you must leave a message to document your cancellation.

The $150 deposit is non-refundable if you cancel with less than 3 business days’ notice.
